• LEAVE OF ABSENCE (LOA) REQUEST 

    Employees seeking to use FMLA leave are required to provide 30-day advance notice of the need to take FMLA leave when the need is foreseeable and such notice is practicable. If leave is foreseeable less than 30 days in advance, the employee must provide notice as soon as practicable – generally, either the same or next business day.

    When an employee is absent ten or more consecutive workdays for medical reasons, the District shall require the employee to request a leave of absence.

      

    LEAVE OF ABSENCE (LOA) FORM AND MEDICAL CERTIFICATION SUBMISSION PROCESS:

    1. For Self: Employee must complete the LOA Form & employee's health care provider completes and signs the Medical Certification:  Employee's Serious Health Condition (Form WH-380-E).  Employee must email both documents to leaves@fwisd.org. 
    2. For Family: Employee must complete the LOA Form & family member's health care provider completes and signs the Medical Certification: Family Member's Serious Health Condition (Form WH-380-F). Employee must email both documents to leaves@fwisd.org.
    3. Maternity Leave: Submit the LOA Form & Verification of Birth Facts within five days after the birth. If leave commences prior to the birth, employee must submit the LOA Form and the Employee's Serious Health Condition (Form WH-380-E) indicating the beginning date of the leave, completed & signed by the health care provider to leaves@fwisd.org.

     

    Important Information:

    • In order to be eligible to take leave under the FMLA, an employee must have worked 1,250 hours during the 12 months prior to the start of leave (includes only those hours actually worked. Paid & Unpaid Leave, Including FMLA leave, are not included) and have worked for the employer for 12 months.
    • Employees seeking to use FMLA leave are required to submit a Medical Certification issued by a health care provider.  If an employee does not provide either a complete and sufficient certification, the employee's request for FMLA leave may be delayed or denied.  If the employee does not provide a Medical Certification, then the leave is not FMLA leave. 
    • When leave is taken intermittently for planned medical treatment, the employee must work with their supervisor and make a reasonable effort to schedule treatment so as not to unduly disrupt the District's operation.
    • All District employees taking FMLA Leave must use the automated absence reporting system to report all absences as soon as the need for the absence is known. Click on the following link: or call 817-814-7827 for assistance.
    • An employee granted leave for a personal medical reason must receive clearance from the Health Services department before returning to work. The employee must have a health-care provider’s written statement or release confirming the specific dates of the illness, restrictions, if any, and the date the employee may return to work. Failure to provide documentation will result in loss of pay.
