• Welcome to the New Grant Questionnaire, a requirement for Fort Worth ISD schools and departments that are applying for a grant for funds, materials, or services from an organization outside of ÁùºÏ²Ê×ÊÁÏ´óÈ«.

    If you are soliciting donations from a funder or if a donor has approached you with funding, please contact Mike Mandl at michael.mandl@fwisd.org or 817.814.2149 for assistance.

    Before you begin the questionnaire, please gather the following items (if applicable):

    • Request for Proposal (RFP) from the funder
    • Contact information for the funder
    • Matching requirements
    • Partner information

    Your responses will be forwarded for review and approval by District leadership, and you will receive notification upon approval. In the meantime, please plan your program and draft your proposal so that you will be ready to meet application deadlines.     By submitting the questionnaire, you acknowledge that as the grant owner or coordinator, you are responsible for maintaining compliance with all grant requirements and guidelines, not limited to reporting deadlines, allowability of activities and expenditures, staffing records including job descriptions, background checks, coordination of the District’s response to audit requests, timely response to grantor requests, maintenance of reporting data, and anything else listed in the contract, program guidelines, or notice of grant award (NOGA).
